Imagine a home like, non-institutional long-term care environment where individuals can move freely and meaningfully participate in the village life and their care. Inspired by the ‘dementia village’ model, in the Netherlands, the goal is to offer first in Canada publicly funded long-term care model for everyone, including those suffering from dementia. This is a big shift away from the traditional service model to a social-relational model of care that puts a person at the centre of it all.

Construction is underway now and the new site is expected to be move in ready in 2024. This new model of care is informed by research, championed by the Conconi Family Foundation. We believe first hand experience should drive the change. The Centre for Health and Evaluation Outcome Science (CHEOS)
