As a member of TransForm, CFF has worked to put ICC on the health and care agenda in Europe and Canada. We are thrilled that this work was a central topic last September at the EPFC 2022. Check out the video to see why.
Through collaborations with a variety of stakeholders, the TransForm Partnership formed the 7 effectiveness principles to outline the key aspects of ICC.

  • Value lived experience, and the community to drive participation and the co-production of care.

  • Foster and build local collaborations to enhance communication and partnerships and drive the implementation of a relationship-based, place-based system of care.

  • Strengthen primary care through expanding the standard definition to be more goal-oriented and focused on prevention. Encourage the co-production of care plans between health providers and the individuals receiving the care to ensure individuals are treated as a whole in their communities.

  • Focus on improving public health by incorporating social, economic, and environmental determinants of health into programming and initiatives. SpaceSSpacepace
  • Support community inclusive solutions by investing in social care and social infrastructure to increase the collaboration and participation of community members.

  • Develop and implement upstream system changes to promote and support the co-creation of care with members of the community.

  • Evaluate the health and wellbeing of the community, and the quality of the services provided on an ongoing basis. Adjust and adapt policies and programs when learnings arise.
