WHAT DO WE MEAN:

  1. Prioritize a relationship-based and place-based system of care
  2. Incorporate the local expertise of community based organizations in developing health and social care
  3. Support geographically bound community alliances as opposed to issue or disease labels (For example, homelessness, addiction, and mental health)

WHY DOES ICC MATTER:

As we saw through the COVID-19 pandemic, our health is interconnected. We’re social beings, and bound to each other, whether we like to admit it or not. We need to expand our frame of care from problem solving disease management to focus on the person as a whole and community well-being. Community expertise is an asset who’s values should be incorporated into the formal system of care.
