What do we mean:

  1. Go beyond Integration  – Moving away from system-centric to person-centric care
  2. Redefine the roles of all stakeholders with a bigger emphasis on individual lived experience, and community as an asset to health and social care
  3. Create new structures that promote co-production of care at the community level, including care recipients

Why does ICC matter:

The current system has been stress tested, and cannot, on its own, withstand the whirlwind of pressures it’s facing, including  social, economic,  demographic, labor, inflationary, and environmental challenges.

Parts of our current system must transition to another way of working. ICC represents one promising model. Under ICC, the care recipient and the community become an integral part of the solution, and help alleviate pressures placed on the health care system.
