TransForm, the Transnational Forum on Integrated Community Care is seeking interested organizations, collaboratives, or networks leading initiatives that align with ICC’s vision and principles to join a learning community that is invested in taking integrated community care forward.

Background

In the summer of 2020, the TransForm partnership [which Conconi Foundation is part of] published a paper titled ICC 4 All. Integrated Community Care for all, is a product of a two-year international work of learning about and defining the concept for better care going forward. It is both a set of principles as well as a movement towards a better system of care. The TransForm partnership worked with groups ranging from individuals with lived experience, families, grassroots organizations, healthcare providers, to policymakers in order to understand and envision what type of shift needs to take place. This report sets the stage for the Changemakers Forum and will serve as the basis for evaluating applications.

The What and How and Why of ICC

Integrated Community Care encompasses a range of Strategies to support [health] professionals, [non-profit] organizations, policy-makers, and community members in a continuous process of co-developing health, care, and social support infrastructures and services with the aim to enhance the quality of life, social cohesion, and resilience of a territorially defined community.

What Now

Given the year behind us, we believe it is especially timely to refocus on Integrated Community Care. Today, we are announcing a call for proposals to join us in taking this work one step further through the Changemakers Forum. The application deadline is May 20, 2021.
